Scope of Services &
Limitations

Mindful Paths Psychological Testing provides diagnostic evaluations within a medical model. We do not offer assessments for:

  • Legal, custody, or disability determinations
  • Court-mandated evaluations
  • Non-clinical validation of specific diagnoses

Evaluations must meet medical necessity criteria. Misrepresentation of testing intent may result in full out-of-pocket charges and forfeiture of insurance coverage. Choosing to test with Mindful Paths may result in waiving your right to future forensic evaluation.

Confidentiality Agreement

All psychological services are confidential. Information will only be released with written consent or as required by law.

Exceptions include:

  • Risk of harm to self or others
  • Suspected abuse (child, elder, dependent adult)
  • Legal mandates
  • Unpaid balances sent to collections
  • Parental access in custody cases
  • Internal consultation (de-identified)

Appointment Policies

  • Initial Evaluations: 60 minutes
  • In-person testing sessions: 4-6 hours. Remote testing options coming soon for patients 25+ miles away.
  • Follow-Ups: 30–60 minutes (therapy), 15–30 minutes (medication)
  • Arrival: Please sign on or arrive 15 minutes early
  • Returning after 1 year: Requires re-evaluation

Cancellation & No-Show Policy

  • Cancellations must be made at least 48 hours in advance
  • Late arrivals may be rescheduled at the provider's discretion
  • No-shows and late cancellation fees are not covered by insurance
  • After three no-shows per year, services may be discontinued (does not apply to MassHealth patients)

No-Show / Late Cancellation Fees

  • Telehealth (Intake, Therapy, Feedback): $100
  • In-person Testing: $300

Emergency
Protocols

Mindful Paths Psychological Testing is an outpatient clinic and does not provide emergency services. If you are in crisis or experiencing suicidal ideation, call 911 or go to the nearest emergency room. After receiving care, follow up with your provider regarding any changes made in the hospital.

Forms & Documentation Requests

  • All requests for letters, forms, or disability paperwork must be discussed with your provider
  • Completion time: 5–7 business days, up to 14 days in some cases
  • Document fees may apply and are the responsibility of the client

Provider Switching Policy

  • Clients may switch providers if their current clinician is unavailable
  • Switching more than twice is limited unless clinically justified
  • Requests based on disagreement with treatment or reports may not be accommodated.

Patient Conduct & Termination

Patients may be discharged from care for:

  • Noncompliance with treatment
  • Repeated attendance violations
  • Aggressive, violent, or inappropriate behavior
  • Breach of clinic policies
